Use this form to request an appointment with us. While we will do our best to accommodate your requested day and time, please note, your appointment is not fully booked until you get a confirmation from us!

CHS Cancellation Policy

• Your appointments are very important to us! Please understand that your appointments are held just for you. If a schedule change arises and you need to cancel or reschedule, we do require a 24-hour notice.
• If your notice is less than 24 hours, you will be charged a deposit of 50% of your service to reschedule. (Except in cases of an emergency)
• NO SHOWS will be charged 100% of service amount.

As a courtesy, we do call, text and/or e-mail to confirm at least 24 hours prior to every appointment. If we are unable to reach you, please understand that it is your responsibility to keep track of your appointments.

We will always try our best to accommodate you if you’re running behind, stuck in traffic, etc. Clients will generally be allowed a 10 minute grace period, however, tardiness affects the remainder of the schedules for the day and you MAY have to forgo parts of the service to keep you in the time allotted for you. Please call as soon as you feel you may be late so we can schedule accordingly.
